Please provide the following on the request form or email:
1. Your in spirit pet's name
2. Date your pet transitioned into spirit
3. Your address where you lived with your animal
4. Address where your animal was when it transitioned(i.e. vet's office - please provide name of hospital and address)
5. A 30 minute session includes up to 3 questions for me to ask
6. A 45 minute session includes up to 5 specific questions for me to ask
7. You may provide a photo if you choose.

NOTE: A session includes any portion of my time I am involved with your session, beginning with and including: all phone calls, reading your session request form, getting additional information from you if necessary, the actual session time with your animal and all administrative functions, payments and emails.
The session time is used to ask your animal questions with the focus on what you want to know based on what you put in your communication request form. I encourage you to allow me the majority of session time to talk with your animal, because your animal has the information you desire.
